The Medical Information ID is made using a 30 ml plastic card material. A professional resin thermal encoder machine is used in printing the ID to create a professional looking, durable and long lasting card. One side of the card has an embedded hologram.
The ID will fit up to 300 characters which can be: text, spaces, or symbols. You could have specific medical instructions, lost & found information, care giver instructions, travel instructions, etc. on the ID card.
If it is a Service Dog ID, we will put the ADA information (American with Disabilities Act) on the back of the ID. This information addresses the issue of permitted access to public buildings.
Note: A dog that is therapeutic to its disabled handler is a Service Dog and has public access rights under the ADA law to enter public property with their disabled handler. Therapy Dogs do not have ADA public access rights. A Therapy Dog is one that meets the needs of others (such as visiting people in nursing homes, hospitals, reading groups, etc.).
The slot at the top allows you to: 1) Place it through the snap hook that is on our cape or padded harness vest. 2) You can purchase a plastic & metal clip or split ring clip if you want to attach the ID to a bag, coat, collar, etc. 3) You can purchase a lanyard and hang the ID around your neck. 4) Place the ID into the clear vinyl window pocket that is on many of our vests.
To send us your dog’s photo and your information, click on the ID Information Form. Photo shots of your dog's head are best, but we will edit (as best we can) another photo.
