The Medical Information ID is made using a 30 ml plastic card material. A professional resin thermal encoder machine is used in printing the ID to create a professional looking, durable and long lasting card. The card is laminated on both sides for extra durability.
The ID will fit up to 300 characters which can be: text, spaces, or symbols. You could have specific medical instructions, lost & found information, care giver instructions, travel instructions, etc. on the ID card.
Service Dogs: A person must be legally disabled and their dog must perform a task for them. The handler has public access rights with a Service Dog and they are protected under Federal ADA Law, and are allowed access to public property with their Service Dog. Emotional Support Dogs:A person must have a note from a licensed mental health professional in order to legally have an ESA dog. These dogs provide an increased comfort level for their owner, particularly in social situations. These dogs do not have ADA public access rights but they are allowed to fly with their owners and qualify for no-pet housing. Therapy Dogs:A trained dog that meets the needs of others such as; visiting nursing homes, hospitals, schools, etc while doing pet therapy work. The handlers do not have ADA public access rights with Therapy Dogs and must have permission from an establishment to enter.

Place it through the snap hook that is on our cape or padded harness vest.
You can purchase a plastic & metal clip or split ring clip if you want to attach the ID to a bag, coat, collar, etc.
You can purchase a lanyard and hang the ID around your neck.
Place the ID into the clear vinyl window pocket that is on many of our vests.
To send us your dog’s photo and your information, click on the ID Information Form. Photo shots of your dog's head are best, but we will edit (as best we can) another photo. This card will be entered in the Activedog's Data Registry. Activedogs.com is not a certifying agency, nor do we accept any responsibility for information that you have us put on your ID badge.

Great ID! High quality ID, I love that it lists a link to the ADA laws. I put my allergy info and a note to not separate my dog from me in an emergency.
Worth the money and they cropped the picture of my dog just right.
